Appointments

Spa visits are by scheduled appointments only. THERE IS A 10 MINUTE GRACE PERIOD ALLOWED FOR ALL APPOINTMENTS. Clients who are late for any appointment may be asked to reschedule at the owner's discretion or will be worked back into the schedule if possible. If client is late and staff agrees to proceed with appointment, a deduction of time will be applied to appointment.

Cancellations

If a client is unable to keep an appointment, we ask that they cancel at least 24 hours in advance. If this is not possible, call as soon as you can so that another client can be given your appointment time.

Late Cancellations

Late Cancellations will be considered a "no-show" and will be billed a $25.00 cancellation fee and will be charged if the appointment is not cancelled within 24 hours.

Deposits and Payments

**ALL DEPOSITS, DOWN PAYMENTS AND PRE-PAID PAYMENTS ARE NON-REFUNDABLE AND NON-TRANSFERABLE FOR 360 body curve LLC.

Missed Appointments (Non-Cancelled)

We understand that occasional missed appointments can occur for a variety of reasons. We track missed (non-cancelled) appointments. A "no-show/late cancellation" is defined as missing an appointment without cancelling at least 24 hours before scheduled time. There will be a charge of $25.00 for a missed or non-cancelled appointment. We appreciate our clients and are diligent in accomodating scheduling based upon their needs to reach their desired goals. Therefore, if any appointments are missed in two consecutive weeks, all payments, including paid packages/deposits are forfeited due to non communication.

Acknowledgment of Cancellation/No Show Policy

A "no-show" is someone who misses an appointment without cancelling in 24 hours of the scheduled appointment. The late cancellation or "no-shows" will be billed a $25.00 cancellation fee or missed appointmens fee.

To acknowledge that I have read the above notice and understand that if I fail to give the required notice cancelation, I will be billed a $25.00 cancellation/missed appointment fee.

NO CHILDREN WILL BE ALLOWED during the client's office visit due to medical liability and risk. If clients bring children to the appointment they will be rescheduled, and a cancellation fee charged
